How it’s structured

Everything in FieldProof fits into four simple parts.

1SurveyA form with a name, a goal and a target number of responses.
2QuestionsWhat you ask. Text, numbers, choices, yes/no, dates, locations and files.
3ResponsesEach completed form, with who collected it and when. Works offline.
4ResultsCharts, completeness scores and a printable report.

How to use it

Pick your role

Use the menu at the top: Manager, Collector or Viewer. Each sees only what they need.

Build or choose a survey

Start from a template, build your own, or import a CSV spreadsheet.

Collect responses

Answer one question at a time. No signal? It saves on the phone and uploads later.

Read the results

Open Results for progress and quality checks, then Report to print or export CSV.

Customer feedback

Measure how satisfied customers are, branch by branch.

Good for: Shops, clinics, banks, service desks

What you get: Average rating, the share who would recommend, answers by branch, and which collectors leave forms incomplete.
